Failed 11.1 install (with pc-install.log)

Hi everyone,
Tried installing 11.1 and finally got it to install, but then it wouldn't boot up. Tried a re-install and could NEVER get it to go. I tried formatting the drive (sd card) but nothing will recognise it. I'm thinking the SD card went bad. What is everyone else's thoughts?

Deleting all gparts
Running: gpart destroy -F /dev/da3
gpart: Permission denied
Running: zpool labelclear -f /dev/da3

Running: gpart create -s gpt /dev/da3
gpart: geom 'da3': File exists
EXITERROR: Error 1: gpart create -s gpt /dev/da3
Unmounting: /mnt
Running: umount -f /mnt

Unmounting DVD/USB media: /cdmnt-install
Running: umount -f /cdmnt-install
